Out of 1,187 skilled nursing homes in California, Town & Country has been ranked 13th, according to the Centers for Medicare and Medicaid Services (CMS). That puts us in the top 1 percent! Nationally, this means we are ranked in the 91st percentile.
About the rating: The overall rating from CMS is based on measured data that’s publicly reported on the CMS Nursing Home Compare tool. The tool combines up to 51 measures across seven aspects of quality—including patient experience and effectiveness of care—into a single star rating for each nursing center. Participating nursing centers report data to CMS, the federal agency that runs the Medicare program, through the Hospital Inpatient Quality Reporting Program. Those centers are given an overall rating that ranges from one to five stars, with five as the highest score and four the most common score. Town & Country has consistently achieved a Five-Star Rating—year in and year out.
About the award: Town & Country has received Top Tier Recognition for it efforts in the 2020 Quality Initiative Recognition Program, run by the American Health Care Association/National Center for Assisted Living.
The Quality Initiative Recognition Program was designed to recognize AHCA nursing center members that demonstrate the attainment of four or more of the Quality Initiative Goals set by the AHCA.
The Quality Initiative is a national effort designed to build upon the existing work of the long-term and post-acute care profession. The initiative aims to further improve quality of care in skilled nursing centers and assisted living communities by challenging members with specific, measurable targets. Of particular focus for AHCA members is to achieve lower re-hospitalizations and reduced usage of antipsychotic drugs. “Best Nursing Homes in California” List by U.S. News and World Report for 2019-2020 and “Best Long-Term Care in California” List by U.S. News and World Report for 2019-2020
About the awards: U.S. News and World Report evaluated more than 15,000 homes nationwide—in every state and nearly 100 major metropolitan areas. This year, 2,969 nursing homes earned the designation of a U.S. News Best Nursing Home. Town & Country Skilled Nursing was one of them.
This year, U.S. News updated its methodology of evaluating nursing homes so that the designation of Best Nursing Home is awarded only to those homes that satisfy U.S. News’s assessment of the appropriate use of key services and consistent performance in quality measures.
To be eligible for a U.S. News overall rating, homes needed to receive either a Short-Term Rehabilitation Rating or a Long-Term Care Rating. Homes rated High Performing in short-term rehabilitation, long-term care or both received a Best Nursing Home designation. Town & Country Skilled Nursing received both distinctions.
