At Grace Gardens Memory Care, residents live as independently as possible

With each new resident, Grace Garden’s expert staff conducts a comprehensive assessment, with an emphasis on understanding their cognitive abilities, special talents and preferences, as well as challenges that might impact function.

Each neighborhood’s programming is attuned to its residents and founded on peer-based, stage-informed care principles. Everything about the household design—from activities, floor plans and interior décor to staff ratios and specialized training—reflects important information about each neighborhood’s peer group.

Two key elements that define the peer group within each household are dementia stage and medical needs. Of course, we also keep personal preferences in mind. What shared personality traits, habits, interests and routines among the residents can add to the social fabric of the household? Integrated together, it fosters a collective, congenial culture.

At Grace Gardens Memory Care, we’ll ask for in-depth information about your loved one so we can design our care around his or her interests, habits and routines. The more we know, the better we can serve.

For example, if your loved one likes to sleep until 9 AM and have a light breakfast, the care staff will be instructed not to wake them any earlier, and the care plan will reflect their preference for a light breakfast. If they like to get up when the sun rises and walk in the garden, we can accommodate that as well.

Additionally, we will learn about favorite activities and what seems to make your loved one content, fulfilled and positive. In this way, we help create the best day we can.

  • Activities will evolve as we learn about the residents. We also offer planned programming, including:

  • One-on-One iPod Music Therapy
The restorative effect of music therapy vastly improves well-being and helps to spark memories and personal preferences.

    Art Therapy
Art therapy enables those with dementia to continue to express themselves and share memories through painting with watercolors.

    Horticultural Therapy
Gardening and plant-based activities get residents outdoors to enjoy the sunny weather, breathe in the fresh air and plunge their hands into the soil.

    Physical Fitness
Strength, balance and exercise trainings keep residents healthy and strong. Learn more about the Grace Gardens Wellness Program.

The perfect solution for married couples: Town & Country and Grace Gardens together

Beyond Grace Gardens Memory Care, we offer independent living, assisted living, skilled nursing and short-term rehabilitation at Town & Country. For couples, this means they can remain close to one another, with one at Grace Gardens and the other within the same community at Town & Country. They can join each other for meals, a walk in the garden or daily activities. We look after any memory care needs, so they can simply enjoy each other’s company.

Experience how it feels to have dementia: take a virtual dementia tour

Our understand firsthand what it’s like to watch your loved one change as they are affected by the dementia. It can be difficult to understand the changes in behavior, which may make the relationship strained. One way to “walk in your loved ones’ shoes,” is to go through the Virtual Dementia Tour, which was developed by Second Wind Dreams.

The participant is outfitted with temporary, patented devices to mimic the common challenges those with dementia face in regard to disorientation, hearing impairment, cognition and dexterity. We are happy to provide this experience to resident family and friends at no charge.
