When Town & Country’s former CEO Dirk DeWolfe started his career in the 1980s, he encountered a dementia resident who just needed to walk. As long as he was walking, he was happy. That formative encounter contributed to the unique physical design of Grace Gardens Memory Care, which provides residents with access to beautiful, natural surroundings, sunlight and plenty of secure, open space to move around.
As a nonprofit organization, Town & Country had the unique opportunity to build a thoughtfully designed memory care community. We believe Grace Gardens will quickly become a vital resource for Orange County residents and their loved ones living with a dementia diagnosis. We are thankful to our donors who have provided the support and encouragement we needed to embark on this journey, and to our voluntary board of directors for creating a culture of generosity.

Just as society has learned to accommodate other kinds of disabilities, Grace Gardens has shaped our environment around those living with dementia to make life easier. We also design our programming to address the individual. We believe there are many shades of cognitive impairment, so we start by investigating possible causes. Is it a drug combination? A stroke? Urinary tract infection? Or Alzheimer’s? We then apply all the tools, best practices and knowledge we have available to improve the physical, emotional and spiritual experience of each resident based on their needs and potential.
Our leadership team are all Dementia Care Specialists, certified by CPI. Each employee receives in-depth dementia care training, followed by ongoing mentoring on a regular basis. Moreover, our leadership team must personally sign off on each employee’s competency and ability to implement our memory care model before they can begin working with our residents.
Grace Gardens Memory Care is staffed 24 hours a day, seven days a week by a licensed nurse. Besides improving overall care, this also often reduces the need for unnecessary hospitalizations, medical tests, and transportation, which can cause undue stress and confusion for residents.
Because Town & Country is based on Christian principles, we also offer a full-time chaplain to attend to the spiritual needs of our residents and families, including prayers, comfort, readings, stories and support.
